**Responsibilities**:

- Collaborate with patient’s doctor for referrals, notes, and medical history.
- Review and evaluate patient through tests, movements, and conversation.
- Create and implement treatment care plans tailored to each patient’s needs.
- Establish patient goals and benchmarks.
- Discuss patients’ goalposts with patients’ physicians, surgeons, and healthcare team.
- Schedule exercises and therapies for the patient.
- Observe and review patient progress.
- Assist and demonstrate proper stretches, techniques, exercise moves, and equipment uses.
- Educate patient family members, caretakers, or close friends on the patient’s treatment plan.
- Recommend specialists to patients who need additional medical attention.
- Document patient medical progress and treatment; record notes to update medical conditions.
- Perform other duties, treatment plans as assigned by the Department Head / Line Manager.

**Skills**:

- Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability.
- Effective listening, observation and facilitation skills to effectively assess patients and provide appropriate treatment and consultation.
- Strong mediation, negotiation and conflict resolution skills to respond or assist in emergency situations.
- Strong collaboration skills to manage service delivery in conjunction with health care professionals, clients, their family members/caregivers/substitute decision makers.
- Ability to deal with demanding interpersonal situations and respond with good judgment and understanding
- Ability to communicate information effectively through a variety of means including meetings, reports, letters and presentations
- Advanced oral and written communication skills in English to establish and maintain a wide range of contacts with health care professionals and community organizations.
- Demonstrated ability to analyze and make independent decisions.
- Ability to think critically and be innovative to create/modify strategy or intervention techniques to suit the individual needs of patients.
