Taux horaire : $35 et plus/heure

T ches et responsabilit s :

  • Recueillir toutes les donn es jug es pertinentes concernant les caract ristiques et les besoins des usagers
  • Proc der l valuation des usagers en utilisant les techniques appropri es et r diger les rapports n cessaires
  • Agir titre d intervenant direct ou indirect en consultation et en orientation des usagers
  • Recommander l orientation g n rale du plan d intervention adapt aux besoins des usagers en prenant en consid ration les ressources de l tablissement
  • Contribuer fournir un support clinique et assurer une fonction conseil aupr s de ses pairs;
  • Agir titre de personne-ressource et de support professionnel aupr s des pairs rencontrant des difficult s particuli res lors de l tablissement du plan d intervention ou lors de la r vision du plan d intervention
  • valuer, selon les m thodes tablies, les r sultats atteints par le plan d intervention et y recommander les ajustements n cessaires, s il y a lieu
  • laborer et animer des programmes d activit s sp cifiques aux besoins rencontr s au niveau de la client le
  • Agir comme personne-ressource lors de consultation et de recherche li es sa profession
  • Participer aux discussions de cas dans l tablissement au besoin et selon les orientations sp cifiques de certains clients
  • D velopper et maintenir les liens de collaboration avec les autres organismes du minist re de la Sant et des Services sociaux, les groupes b n voles ou autres organismes communautaires int ress s la probl matique
  • Participer des rencontres avec le chef de service, l quipe et lorganisation

Exigences :
  • D tenir un baccalaur at en sciences humaines ou sciences sociales tel que criminologie, travail social ou psychologie
  • Maitrise de la langue fran aise (anglais un atout)
  • D tenir un permis dordre professionnel
  • D tenir un permis de conduire valide
  • Poss der un v hicule
