Clinical Pharmacist

JOB SUMMARY: The Clinical Pharmacist - I will be responsible for providing pharmaceutical care services to patients within Sidras healthcare facility. The post holder collaborates with healthcare professionals to optimize medication therapy and ensures safe and effective medication use. KEY ROLE ACCOUNTABILITIES: • Conducts comprehensive medication reviews to assess appropriateness, effectiveness, and safety of medication therapy. • Develops and implements pharmaceutical care plans for individual patients based on their specific needs and medical conditions. • Provides drug information to healthcare professionals, patients, and caregivers. • Monitors patient outcomes and adjusts medication therapy as necessary to achieve desired therapeutic goals. • Participates in multidisciplinary healthcare teams to develop and implement treatment protocols and guidelines. • Ensures compliance with regulatory requirements and standards of practice in pharmacy operations. • Provides education and training to pharmacy staff, healthcare professionals, and patients on medication-related topics. • Maintains accurate and complete patient records and documentation. • Stays abreast of developments in pharmacy practice, pharmacotherapy, and healthcare trends. • Adheres to Sidra’s standards as they appear in the Code of Conduct and Conflict of Interest policies • Adheres to and promotes Sidra’s Values QUALIFICATIONS, EXPERIENCE AND SKILLS: ESSENTIAL PREFERREDEducation • Bachelor of Pharmacy plus • PharmD or master's degree in clinical pharmacy or its equivalent All degrees must have been obtained from an ACPE accredited college of Pharmacy or equivalent Experience • 2+ years of post-graduate experience as a licensed registered Clinical Pharmacist in a, accredited hospital setting. (Pharmacy internship period is not counted in the experience) • 0 experience as fresh PharmD graduate from Qatar University Clinical pharmacy experience in a pediatric hospital settingCertification and Licensure • Licensed in Pharmacy in country of origin • Obtain State of Qatar Pharmacy License after on boarding.
