Position Overview

We are seeking a Patient Care Coordinator to serve as the first point of contact for our patients, ensuring they feel valued and supported throughout their journey with us. This role is ideal for someone with excellent customer service skills, a proactive approach to problem-solving, and acutely strong attention to detail. You need to be able to foresee problems before they happen and take active steps to prevent them from happening. A systems-based approach to workflows and customer service is required.

The Patient Care Coordinator will be responsible for building relationships with our patients, following up with them regularly, and ensuring they receive the highest level of personalized care. Additionally, this role involves managing provider scheduling, including coordinating provider availability for events and effectively communicating scheduling details. You need to be a team player that is truly invested in fostering a proactive approach and leading the customer service offering at Seed, working collaboratively with management, patients, and colleagues, ensuring seamless communication, compassionate care, and a solutions-oriented mindset that upholds Seed's mission and values.

Key Responsibilities

  1. Provide a warm and professional welcome to patients, ensuring a positive and personalized experience.
  2. Build strong relationships with patients by following up on their care and wellness journey.
  3. Coordinate scheduling for all providers, ensuring coverage for events and appointments.
  4. Communicate scheduling details with providers and coordinate logistics for their availability.
  5. Anticipate and proactively address potential scheduling conflicts or operational challenges.
  6. Work closely with the clinical team to ensure smooth clinic operations.
  7. Uphold a high standard of patient service, resolving inquiries and concerns with empathy and efficiency.
  8. Assist with administrative tasks as needed, supporting the overall workflow of the center.

Qualifications & Skills

  1. Experience: Background in hospitality, customer service, or healthcare administration preferred.
  2. Attention to Detail: Ability to anticipate scheduling challenges and proactively find solutions.
  3. Scheduling Expertise: Strong organizational skills with the ability to manage and optimize provider availability.
  4. Communication & Interpersonal Skills: Professional, friendly, and able to engage with patients and team members effectively.
  5. Initiative & Problem-Solving: A proactive approach to identifying and resolving challenges.
  6. Team-Oriented Mindset: Enjoys working collaboratively in a dynamic environment.
  7. Flexibility: Willingness to work flexible hours as needed to support the clinic's operations.
  8. Passion for Health & Wellness: A demonstrated interest in wellness, holistic health, or patient care.
